Chautauqua County Health Network awarded $200,000 federal grant

U.S. Rep. Tom Reed (R-NY) announced this week that the Chautauqua County Health Network (CCHN) in Jamestown, New York has been awarded a $200,000 grant from the Health Resources and Services Administration.

“Ensuring local access to health care remains one of my top priorities,” Reed said. “I continue to fight to ensure that our rural hospitals have access to the resources they need to provide these essential services.”

The grant funds will be utilized by CCHN for equipment and education designed to combat chronic heart failure, chronic obstructive pulmonary disease and type 2 diabetes in patients.

“On behalf of the Chautauqua County Health Network, we are thrilled to receive this Rural Healthcare Coordination Network Partnership program funding to help health care providers in our network expand their care coordination activities to better serve patients burdened by chronic disease and utilize the proven best medical practices available,” Ann Abdella, the executive director of the CCHN, said. “We look forward to building stronger partnerships between Chautauqua County hospitals, primary care providers and hospice.”

CCHN is made up of three hospital organizations, and their governing boards and medical staff. It also has two sister-organizations: the Integrated Delivery System and Chautauqua Region Associated Medical Partners.
